Nurse Practitioner – Primary Care | California

Location: Sunnyvale, California

Schedule: Monday to Friday, 9 AM – 5 PM

Compensation: $180,000.00 - $200,000 per year + benefits

We are seeking a dedicated, patient-focused Nurse Practitioner (NP) to join our expanding outpatient practice in California. This is a dynamic opportunity for a licensed Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) to work at the forefront of integrated, team-based care—delivering high-quality primary care to a diverse population of patients.

Whether you're an experienced NP or a recent graduate with strong clinical rotations, we’re looking for someone passionate about whole-person care and building long-term patient relationships.

Your Responsibilities:

  • Perform comprehensive health assessments, including histories and physicals
  • Diagnose and treat acute illnesses, manage chronic diseases (diabetes, hypertension, asthma, etc.)
  • Order and interpret labs, imaging, and diagnostic tests
  • Conduct wellness visits, immunizations, and patient counseling
  • Develop personalized care plans in collaboration with patients and clinical team
  • Document visits and coordinate care using a fully integrated EMR system
  • Educate patients on disease prevention, lifestyle changes, and self-management
  • Refer patients to appropriate specialty providers when necessary
  • Participate in team meetings, QI initiatives, and population health management

Requirements:

  • Master’s or Doctorate degree from an accredited Nurse Practitioner program
  • Certification as FNP or AGNP (AANP or ANCC)
  • Active, unrestricted California Nurse Practitioner license
  • Furnishing number and DEA license (or eligible to obtain)
  • BLS/ACLS certification
  • Passion for community care, underserved populations, or preventive medicine
  • Experience in primary care, urgent care, or chronic disease management preferred (but not required)
