Nurse Practitioner – Primary Care | Colorado
Location: Steamboat Springs, Colorado

Schedule: Monday to Friday, 9 AM – 5 PM
Compensation: $180,000.00 - $200,000 per year + benefits

We are seeking a dedicated, patient-focused Nurse Practitioner (NP) to join our expanding outpatient practice in Colorado. This is a dynamic opportunity for a licensed Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) to work at the forefront of integrated, team-based care—delivering high-quality primary care to a diverse population of patients.
Whether you're an experienced NP or a recent graduate with strong clinical rotations, we’re looking for someone passionate about whole-person care and building long-term patient relationships.
Your Responsibilities:
- Perform comprehensive health assessments, including histories and physicals
- Diagnose and treat acute illnesses, manage chronic diseases (diabetes, hypertension, asthma, etc.)
- Order and interpret labs, imaging, and diagnostic tests
- Conduct wellness visits, immunizations, and patient counseling
- Develop personalized care plans in collaboration with patients and clinical team
- Document visits and coordinate care using a fully integrated EMR system
- Educate patients on disease prevention, lifestyle changes, and self-management
- Refer patients to appropriate specialty providers when necessary
- Participate in team meetings, QI initiatives, and population health management
Requirements:
- Master’s or Doctorate degree from an accredited Nurse Practitioner program
- Certification as FNP or AGNP (AANP or ANCC)
- Active, unrestricted Colorado Nurse Practitioner license
- Furnishing number and DEA license (or eligible to obtain)
- BLS/ACLS certification
- Passion for community care, underserved populations, or preventive medicine
- Experience in primary care, urgent care, or chronic disease management preferred (but not required)
