Job Opening: Homecare Physical Therapist (PT)
Location: Woodcliff Lake, New Jersey
Job Type: Full-time/Part-time
Work Hours: Monday to Friday, 9 AM – 5 PM
Compensation: $90,000 - $110,000 per year + benefits (Full-Time)
About the Job:
We’re currently seeking a Licensed Physical Therapist who is passionate about providing personalized care and values the autonomy and flexibility that home health offers.
As a Homecare Physical Therapist, you will evaluate, treat, and support patients across a variety of conditions—post-surgical, orthopedic, neurological, and more. You’ll create custom treatment plans, educate patients and families, and track progress, all while enjoying a flexible schedule and the chance to truly connect with your patients.
What You’ll Do:
- Conduct in-home physical therapy evaluations and develop individualized treatment plans
- Provide therapeutic exercises, gait training, balance activities, and functional mobility interventions
- Educate patients and caregivers on home safety, fall prevention, and wellness strategies
- Monitor progress and adjust plans based on patients’ functional goals
- Accurately document all visits in accordance with Medicare and agency guidelines
- Communicate with the interdisciplinary team (nursing, OT, SLP, social work) for coordinated care
What Makes You a Great Fit:
- Active New Jersey Physical Therapist License (required)
- Graduate of an accredited PT program (DPT or MPT)
- Valid driver’s license, reliable vehicle, and auto insurance
- CPR/BLS certification
- Strong clinical judgment and time-management skills
- Comfortable working independently in a home setting
- 1+ year of experience in home health, outpatient, or hospital-based rehab
- Familiarity with EMR systems (e.g., HomeCare HomeBase, Kinnser, MatrixCare, etc.)
- Knowledge of Medicare documentation and OASIS (preferred, not required—training available)
