Job Types: Full-time

Pay: $80,000 - $90,000 per year + benefits

We are seeking a Full-Time Licensed Independent Clinical Social Worker (LICSW) to join our telehealth team and contribute to helping clients discover healing and develop a sense of mastery over their lives through mindfulness and taking action

Our Clinical Social Workers provide valuable services that help connect home health patients and their families with community resources for social, emotional, financial and personal support (such as transportation and meal programs), associated with illness and recovery. As part of our Medical Social Services Team, you will have the opportunity to improve the psycho-social well-being of our patients and their families.

Responsibilities:

  • Conducts initial assessment of all new patients to evaluate social, emotional, and physical needs by applying social work theories and techniques; interviews and evaluates patients and family members.
  • Identifies factors which may interfere with obtaining maximum benefits from treatment.
  • Develops and implements patient management plans, including the coordination of referrals to specialty and social services, the recording of progress notes, and the provision of continuity of care.
  • Helps clients cope with environmental and psychological issues of illness by explaining options and treatments as well as making appropriate referrals.
  • Collaborate with psychiatrists, counselors, and healthcare providers to ensure integrated care.
  • Advocate for clients by connecting them with community resources and support services.
  • Participate in case reviews, staff meetings, and continuing education opportunities.

Requirements:

  • LCSW - Licensed Medical Social Worker - State Licensure
  • Master's degree in Social Work from an accredited college or university
  • Two (2) years of progressively responsible and directly related work experience
  • Ability to gather data needed to assess the needs of patients and families
  • Ability to provide written case narratives and reports
  • Knowledge of and ability to apply effective assessment, interviewing, and counseling techniques
  • Experience using Microsoft Suite (Word, Excel, Outlook), Google Suite or other systems
  • Excellent written and verbal communication
