Full-time/Part-time Home Health Aide needed at Holistic Home Care in Brookhaven, NY.

Schedule

  • Monday to Friday (Weekend Availability)

Responsibilities

  • Reporting to the Home Care Supervisor

  • Provide personal care and assistance with activities of daily living, including but not limited to bathing, dressing, grooming, dressing, toileting, transferring etc

  • Assist participants with Instrumental Activities of Daily Living (IADLs), including but not limited to laundry, light housekeeping, shopping, etc.

  • Measure and document participants’ vital signs as instructed

  • Aid with light meal preparation and feeding in participants’ homes, ensuring participants are nourished and hydrated

  • Help participants with safe transfer in and out of their homes, and act as medical escort on vehicles and to medical appointments as assigned

  • Assist the Rehabilitation department by carrying out a home exercise program

  • Provide translation services as needed, if qualified

Requirements

  • High School Diploma or Equivalency

  • Current Basic Life Support (BLS)

  • First Aid Certification required

  • Home Health Aide (HHA) certification preferred

  • Bilingual preferred

  • Must be registered with Home Care Registry prior to start date

Salary/Wage

$30000 to $55000 per year + Bonus + Equity
