Full-time/Part-time Home Health Aide needed at Holistic Home Care in Brookhaven, NY.

Monday to Friday (Weekend Availability)
Responsibilities

Reporting to the Home Care Supervisor
-
Provide personal care and assistance with activities of daily living, including but not limited to bathing, dressing, grooming, dressing, toileting, transferring etc
-
Assist participants with Instrumental Activities of Daily Living (IADLs), including but not limited to laundry, light housekeeping, shopping, etc.

Measure and document participants’ vital signs as instructed

Aid with light meal preparation and feeding in participants’ homes, ensuring participants are nourished and hydrated
-
Help participants with safe transfer in and out of their homes, and act as medical escort on vehicles and to medical appointments as assigned

Assist the Rehabilitation department by carrying out a home exercise program

Provide translation services as needed, if qualified
Requirements

High School Diploma or Equivalency
-
Current Basic Life Support (BLS)
-
First Aid Certification required
-
Home Health Aide (HHA) certification preferred
-
Bilingual preferred
-
Must be registered with Home Care Registry prior to start date
Salary/Wage
$30000 to $55000 per year + Bonus + Equity
