Position: Remote Case Manager

We are seeking a skilled and empathetic Remote Case Manager to join our team. This role involves providing virtual case management services to clients and their families while ensuring that their needs are met in a compassionate and professional manner. The ideal candidate should comprehensively understand case management, be an excellent communicator and problem-solver, and work effectively in a remote environment.

Responsibilities:

  • Conduct thorough assessments of client's needs and develop customized care plans.
  • Provide ongoing counseling and support to clients and their families.
  • Work with healthcare professionals, community organizations, and social services to coordinate client care and ensure compliance with treatment plans.
  • Assist clients in accessing community resources and services when required.
  • Maintain accurate records of client care and progress.
  • Provide crisis intervention and support to clients in times of need.
  • Attend team meetings and continuing education sessions to stay informed about emerging trends and best practices in case management.
  • Provide case management services to clients in a secure virtual setting.

Requirements:

  • A Master's degree in social work, psychology, counseling, or a related field.
  • An independent LICSW, LMHC, or LMFT license
  • Minimum of two years of experience providing case management services to clients.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively independently as well as in a team setting.
  • Knowledge of community resources and services.
  • Proficient in using technology for remote work, such as secure messaging platforms, electronic health records, and video conferencing.
  • Must have access to a reliable high-speed internet connection.
