Job Title: Remote Outpatient Caseworker

Location: Remote

Job Type: Full-time, Permanent

Salary Range: $80,000-100,000

We are seeking a compassionate and experienced Remote Outpatient Caseworker to join our team. In this role, you will be responsible for providing case management services to patients receiving outpatient care remotely. The successful candidate will have a strong understanding of outpatient care, excellent communication and problem-solving skills, and the ability to work independently in a remote setting.

Responsibilities:

  • Conduct initial assessments of patients to determine their needs and develop individualized care plans.
  • Provide ongoing support and counseling to patients and their families.
  • Collaborate with healthcare professionals to coordinate patient care and ensure that treatment plans are followed.
  • Assist patients in accessing community resources and services as needed.
  • Maintain accurate and up-to-date records of patient care and progress.
  • Provide crisis intervention and support to patients in times of need.
  • Participate in team meetings and continuing education to stay current on best practices and emerging trends in outpatient care.

Requirements:

  • A Master's degree or higher in social work, counseling, psychology, or a related field.
  • An active and independent LCSW-C, LCPC, LCMFT, or LMFT license
  • Minimum of 2 years of experience providing case management services to patients in an outpatient setting.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Knowledge of community resources and services.
  • Proficient in using technology for remote work, such as video conferencing, electronic health records, and secure messaging platforms.
  • Access to a reliable high-speed internet connection.
