Join Our Team as a Home Health LPN!

We’re looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to provide essential care to patients in the comfort of their own homes. In this role, you’ll follow personalized, one-on-one care plans, administering medical support and building genuine connections with each patient you serve. Your clinical skills, attention to detail, and caring approach will make a lasting impact, helping patients feel valued and supported on their health journey.

Salary: $60K TO $70K a year

Key Responsibilities:

• Provide health-related education as appropriate to patient needs
• Administer medication and complete related pharmacological procedures
• Collect data for assessment of care needs, report findings, and deliver appropriate nursing interventions
• Monitor patient's progress and report outcomes

Qualifications:

• Must be currently licensed as a Licensed Practical Nurse in the State of employment..
• Two years of clinical experience
• CPR Certification
• Valid Driver's License
