Now Hiring: In-Office Case Manager (California)

Salary Range: $100K to $120K a year

Location: Berkeley, California

Job Type: Full-Time

Schedule: Monday to Friday, 9 AM – 5 PM


Our California-based behavioral health and social services clinic is seeking dedicated, in-office Case Managers to join our compassionate team.

Position Summary:

The Case Manager plays a key role in coordinating client services, advocating for needs, and guiding individuals toward healthier, more independent lives. You will work closely with clients in person, providing assessments, referrals, support, and follow-up in a structured, welcoming office environment.

Key Responsibilities:

  • Conduct comprehensive client intakes and needs assessments
  • Develop and implement individualized service and care plans
  • Provide direct referrals and connections to housing, health care, employment, and community resources
  • Advocate on behalf of clients with service providers and agencies
  • Maintain accurate and timely case documentation using EHR systems
  • Coordinate with therapists, social workers, and program directors as part of a multidisciplinary team
  • Monitor client progress and adjust service plans as needed
  • Ensure compliance with privacy regulations and agency policies

Qualifications:

  • Bachelor’s degree in Social Work, Psychology, Human Services, or related field (required)
  • Master’s degree or prior case management experience (preferred)
  • Strong knowledge of community resources and support systems in California
  • Excellent communication, time management, and organizational skills
  • Proficient with documentation and case management software
  • Ability to work on-site and interact directly with clients in the office
  • Bilingual (English/Spanish or other) is a plus but not required
