Mental Health Case Manager – Pleasanton, CA

We are looking for a Mental Health Case Manager who is committed to supporting individuals facing substance use challenges, often with co-occurring conditions. Bilingual candidates fluent in one of BHSD’s threshold languages—Chinese, Spanish, Tagalog, or Vietnamese—may qualify for an additional incentive.
In this role, you will provide essential support and advocacy, helping individuals transition from crisis to stability. You’ll engage in crisis intervention, case management, and collaborative care within a supportive and inclusive work environment. Your efforts will directly contribute to improving lives by connecting people with the resources they need for long-term recovery.
Position Details
- Schedule: Monday – Friday, 9:00 AM – 5:00 PM
- Compensation: $31–$41 per hour (based on experience and qualifications)
- Employment Type: Full-time
Key Responsibilities
- Conduct comprehensive assessments to determine client needs related to mental health and substance use.
- Provide case management services at the Access Triage Center.
- Coordinate and schedule medical, psychiatric, and continuing care appointments.
- Maintain accurate and timely documentation in accordance with case management standards.
- Deliver crisis intervention and de-escalation support when needed.
- Collaborate with Alameda County and Behavioral Health providers to ensure integrated client care.
- Build and maintain strong relationships with community providers and managed care organizations.
- Facilitate case management training during staff meetings.
- Educate clients on available treatment options, including withdrawal management, mental health services, residential treatment, and recovery residences.
- Utilize technology for effective communication and case documentation.
- Attend team meetings, trainings, and contribute to a positive workplace culture.
- Drive program vehicles as needed.
- Perform other duties as assigned by the Program Manager.
Qualifications
- Currently enrolled in a Master’s program working toward licensure and/or registered with the California Board of Behavioral Sciences (BBS) as an AMFT or ASW.
- Experience providing case management services in a mental health or substance use disorder program, particularly in the nonprofit sector.
- Experience working in crisis intervention settings.
- Preferred: Bilingual in English, Spanish, Chinese, Vietnamese, or Tagalog.
