Full Time Licensed Mental Health Therapist (LCSW, LMFT, LPC)

Provides patients and their families with the psychosocial support they need to cope with acute, chronic, or terminal illnesses. Services include patient education and counseling, guidance for family caregivers, and service referrals.

Schedule: Full Time

Your Role :

  • Utilizes the electronic medical record to monitor, document and communicate patient progress toward goals and progression of the social work plan.
  • Collaborates and communicates with an interdisciplinary team anticipating needs to move the plan of care forward.
  • Provides support and information to patients and families regarding transition plans.
  • Maintains knowledge of government and private payer networks and services to assure appropriate transitions.
  • Collaborates with community and health care resources based on the need to coordinate care for the patient.
  • Advocates for patients and families by supporting patient rights and accessing protective services.
  • Demonstrates awareness of patient rights and ethical decision making; provides advocacy to support patients and family.
  • Assures appropriate reporting of vulnerability or suspected abuse as mandated by law.
  • May participate in care system processes that prevent readmissions.
  • Plans and participates in transition conferences with patients and families.
  • Utilizes tools and technology to identify and intervene with patients who are at risk for readmission.
  • Ensures that a complete clinical handoff occurs for at risk patients, which may include referrals.
  • Other duties as assigned.
  • Provides support and counseling to patients and families.
  • Provides information and assistance for identified financial or social needs.

What We’re Looking For:

  • Ability to manage a large caseload, and to interface with large providers and support staff.
  • Proven passionate about impacting lives and the future of behavioral care
  • Efficient use of electronic medical records.
  • Proficient electronic mail communication skills.
  • Master’s or doctorate degree in clinical psychology, counseling, or clinical social work (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD) from an accredited program
  • An independent license in the state where you reside (e.g., LCSW, LMFT, LPC) from the USA
