Full Time Licensed Mental Health Counselor (LCSW,LMFT,LPCC)

In an integrated medical and behavioral health context, the therapist offers patients screening, consultation, behavioral intervention, referrals, counseling, and treatment. It takes a strong team player and self-starter for this role.

Job Schedule : Full-time

Essential Responsibilities:

  • Provide individual and group therapy to promote overall wellness, in accordance with the treatment plan.
  • Consult with medical providers on psychosocial issues patients bring to their medical appointments.
  • Triage both urgent and routine referrals from medical providers
  • Collaborate with staff and providers to schedule patients for intake
  • Engage in care team huddles in primary care departments
  • Formulate client-centered treatment plans for assigned clients
  • Undertake case management, advocacy, medication monitoring, emergency services, insurance companies, education referrals and consultation with other agencies as appropriate
  • Collaborate with primary care, prescribing physician, emergency services, life skills educations and consult other agencies regarding each client
  • Performs other duties as assigned.
  • Provide assessment, evaluation and mental health treatment

Qualifications:

  • Candidates should have (LMFT , LCSW , LPCC)
  • Excellent time management skills; organized and able to prioritize
  • Comfortable in a fast-paced environment with multiple patients
  • Able to organize and manage large amounts of files, schedules, dates, and information
  • Calm and cool-headed with a positive attitude
  • Self-directed and self-motivated
  • Candidates are required to have a master’s degree in one of the behavioral sciences or related fields from an accredited college or university.
