Salary:
$110,000 to $130,000 per year, plus benefits

Responsibilities:

• Assist clients in navigating the behavioral health and physical healthcare systems.
• Provide education on wellness resources to encourage healthy living.
• Attend multi-disciplinary team meetings and contribute to patient care plans.
• Maintain accurate documentation in client records.
• Use evidence-based tools to assess client needs and communicate findings.

Requirements:

• Master’s degree in Social Work, Counseling, or related field.
• Active LCSW license in California.
• Minimum of 3 years of outpatient experience.
• Strong communication skills and ability to work collaboratively.
• Adaptability to a fast-paced work environment.
