Salary:
$110,000 to $130,000 per year, with full benefits

Key Responsibilities:

• Support clients in navigating behavioral health and physical health systems.
• Educate individuals about wellness resources to foster healthy lifestyles.
• Participate in treatment team meetings and care coordination.
• Ensure accurate and thorough documentation of client records.
• Implement evidence-based assessment tools to identify client needs.

Qualifications:

• Master’s degree in Social Work or related field.
• Active LCSW license in California.
• At least 3 years of experience in an outpatient environment.
• Excellent communication and teamwork skills.
• Ability to work effectively in a fast-paced setting.
