Salary:
$110,000 to $130,000 annually, plus comprehensive benefits

Responsibilities:

• Help clients navigate the healthcare system for optimal support.
• Educate on available resources for disease prevention and wellness.
• Attend and contribute to interdisciplinary treatment team meetings.
• Document clinical interactions accurately.
• Use evidence-based tools for assessments and reporting.

Requirements:

• Master’s degree in Social Work or a related discipline.
• LCSW license in California.
• 3+ years of experience in outpatient settings.
• Strong verbal and written communication skills.
• Ability to thrive in a collaborative and dynamic environment.
