Full-time/Part-time Social Worker Needed for Community Center in Eugene, OR

Our community center is looking for a dedicated and compassionate Social Worker to join our team. The ideal candidate is fully licensed with the state board and will provide essential support and guidance to seniors and their families, ensuring their well-being and promoting a high quality of life.

As a Social Worker, you will play a vital role in evaluating the psychosocial needs of our clients and coordinating services to enhance their overall well-being.

Work Hours:

- Monday to Friday: 8:00 AM to 4:30 PM (Full-time schedule)

- Occasional evening or weekend hours may be required based on client needs and availability.

- Flexibility in scheduling is necessary to accommodate visits and ensure continuous care.

- On-call availability may be required to address urgent situations or provide support during off-hours.

- Part-time schedule is based on needs and work hours are negotiable.

Pay:

$60,000 to $100,000 per year

Job Responsibilities:

- Conduct thorough assessments of seniors' psychosocial needs, considering emotional, mental, and social factors.

- Collaborate with clients and their families to develop personalized care plans addressing their specific goals and preferences.

- Offer counseling and emotional support to seniors and their families, helping them navigate life transitions and challenges.

- Advocate for clients' rights and access to community resources, ensuring they receive appropriate support and services.

- Work alongside interdisciplinary teams, including nurses, caregivers, and healthcare professionals, to coordinate and enhance care.

- Conduct regular visits to monitor clients' well-being, assess their living conditions, and address any concerns or safety issues.

- Facilitate communication and resolve conflicts among family members involved in the senior client's care.

- Educate clients and their families about available community resources, support groups, and programs to enhance their social engagement and overall quality of life.

- Maintain accurate and up-to-date documentation of client assessments, care plans, progress notes, and relevant interactions.

- Stay informed about current social work practices, policies, and regulations, ensuring compliance with ethical standards.

Qualifications:

  • Master's degree in Social Work from an accredited institution.
  • Current and valid state licensure as a Social Worker (LCSW)
  • Minimum of two years of experience in a social work role, preferably in a community center or geriatric setting.
  • Thorough knowledge of social work principles, aging-related issues, and available community resources.
  • Excellent assessment, counseling, and problem-solving skills.
  • Ability to empathize, show compassion, and establish rapport with seniors and their families.
  • Strong communication and interpersonal skills, enabling effective collaboration with interdisciplinary teams.
  • Proficiency in using electronic health record (EHR) systems and computer applications.
