Work Schedule:
Your typical work hours will be from 8:00 a.m. to 5:00 p.m., totaling 40 hours per week.
Responsibilities:
- Conduct thorough assessments and diagnose acute and chronic medical conditions.
- Develop personalized treatment plans tailored to each patient's needs.
- Prescribe medications and therapies, and order diagnostic tests as necessary.
- Educate patients on disease prevention, health maintenance, and lifestyle modifications.
- Collaborate with interdisciplinary teams to coordinate patient care and referrals.
- Maintain accurate electronic health records and ensure timely documentation.
- Participate in quality improvement initiatives and adherence to clinical guidelines.
- Stay abreast of current healthcare practices and regulatory standards.
Requirements:
- Current certification as a Nurse Practitioner.
- Active state nursing license.
- Master's degree in nursing from an accredited program.
- Minimum of 3-5 years of clinical experience in primary care.
- Excellent communication and patient engagement skills.
- Strong analytical and problem-solving abilities.
