Full-time Community Health Nurse Practitioner!!
You will have the opportunity to provide holistic care to individuals and families in the community, addressing their unique healthcare needs and advocating for equitable access to quality services. By collaborating with community organizations and implementing health education initiatives, you will play a pivotal role in empowering individuals and improving community health outcomes.
Work Schedule:
You will have the opportunity to provide holistic care to individuals and families in the community, addressing their unique healthcare needs and advocating for equitable access to quality services. By collaborating with community organizations and implementing health education initiatives, you will play a pivotal role in empowering individuals and improving community health outcomes.
Work Schedule:
- Your schedule will range from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m.
- Participation in community outreach events or health fairs during weekends may be required.
- An average workweek totaling approximately 40 hours
- Conduct comprehensive health assessments and screenings for individuals and families in the community.
- Diagnose and manage acute and chronic medical conditions, including preventive care and health promotion.
- Collaborate with community organizations and stakeholders to develop and implement health education programs.
- Provide culturally sensitive care and advocate for patients' rights and access to healthcare services.
- Coordinate referrals and follow-up care to ensure continuity of care for patients.
- Maintain detailed and accurate medical records in compliance with regulatory standards.
- Participate in community outreach events and health fairs to raise awareness and provide health screenings.
- Stay updated with community health trends, evidence-based practices, and relevant policies
- Current certification as a Nurse Practitioner.
- Active state nursing license.
- Master's degree in nursing from an accredited program.
- Minimum of 3-5 years of clinical experience, preferably in community health or public health settings.
- Strong interpersonal and communication skills, with the ability to work effectively with diverse populations.
- Commitment to social justice, health equity, and advocating for vulnerable communities
- Competitive salary range of $150,000-$180,000 a year
