About the Role:
As part of a growing healthcare system in California, we are looking for an experienced Nurse Practitioner to join our team. This role offers an exciting opportunity to make a meaningful impact in patient care through both independent decision-making and collaborative teamwork.Key Responsibilities:
- Conduct thorough initial and follow-up assessments, including obtaining detailed medical histories, performing physical exams, and ordering diagnostic tests as appropriate.
- Diagnose, treat, and manage both acute and chronic conditions in diverse populations while collaborating with physicians for complex cases or specialty consultations. Develop and implement personalized care plans for a wide variety of conditions, such as hypertension, diabetes, asthma, mental health issues, and other common illnesses.
- Prescribe, adjust, and manage pharmacological therapies within the scope of your practice. Monitor patient responses to medications and adjusted treatments accordingly to optimize patient outcomes.
- Provide preventive care services, including immunizations, screenings, and counseling on health promotion and lifestyle modification. Offer patient education on managing chronic conditions, disease prevention, and healthy living strategies.
Qualifications:
- Master’s or Doctoral degree in Nursing from an accredited Nurse Practitioner program
- Valid, active Nurse Practitioner license in the state of California
- Board certification in the appropriate specialty (e.g., Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (AGNP), Psychiatric Nurse Practitioner (PMHNP), etc.
- Minimum of 1-3 years of clinical experience in a similar setting; however, recent graduates with relevant clinical rotations are also encouraged to apply
- Strong communication and interpersonal skills for building trust with patients and working with a healthcare team
