Work Schedule:

  • Type: Full-time position.
  • Days: Monday to Friday.
  • Hours: 8:00 a.m. to 5:00 p.m., with occasional extensions until 8:00 p.m.
  • Additional: Potential participation in weekend events.
  • Weekly Commitment: Approximately 40 hours.

Key Responsibilities:

  • Perform thorough health assessments for individuals and families.
  • Manage both acute and chronic cardiac conditions with a focus on preventive care.
  • Collaborate with community partners to advance health education initiatives.
  • Deliver culturally competent care, ensuring patient accessibility and rights.
  • Facilitate patient referrals and ensure continuity of care.
  • Maintain accurate and compliant medical documentation.
  • Engage in community health events, promoting awareness and providing screenings.
  • Stay abreast of community health trends, evidence-based practices, and policy updates.

Compensation & Benefits:
