Job Type: Full-time

Salary: $150,000 to $160,000

Position Summary

You will have the opportunity to provide holistic care to individuals and families in the community, addressing their unique healthcare needs and advocating for equitable access to quality services. By collaborating with community organizations and implementing health education initiatives, you will play a pivotal role in empowering individuals and improving community health outcomes.

Job Responsibilities:

  • Diagnose and manage acute and chronic medical conditions, including preventive care and health promotion.
  • Collaborate with community organizations and stakeholders to develop and implement health education programs.
  • Provide culturally sensitive care and advocate for patients' rights and access to healthcare services.
  • Coordinate referrals and follow-up care to ensure continuity of care for patients.
  • Participate in community outreach events and health fairs to raise awareness and provide health screenings.

Qualifications:

  • Current certification as a Nurse Practitioner.
  • Active state nursing license.
  • Master's degree in nursing from an accredited program.
  • Minimum of 2-5 years of clinical experience, preferably in community health or public health settings.
