Full-time Community Health Nurse Practitioner

Job Type: Full-time, Monday – Friday, 9-5pm

Salary: $150,000 to $160,000

By collaborating with community organizations and leading health education initiatives, you will play a pivotal role in empowering individuals and enhancing community health outcomes.

Responsibilities:

  • Collaborate with community organizations and stakeholders to develop and implement health education programs.
  • Provide culturally sensitive care and advocate for patients' rights and access to healthcare services.
  • Maintain detailed and accurate medical records in compliance with regulatory standards.
  • Participate in community outreach events and health fairs to raise awareness and provide health screenings.
  • Stay updated with community health trends, evidence-based practices, and relevant policies.

Requirements:

  • Master's or Doctoral degree in Nursing from an accredited program
  • Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) by the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP)
  • Current, unrestricted Nurse Practitioner (APRN) license in New York State
  • DEA# and NPI# are required
  • Strong clinical assessment and diagnostic skills
  • Minimum of 3-5 years of clinical experience, preferably in community health or public health settings.
  • Strong interpersonal and communication skills, with the ability to work effectively with diverse populations.
