Job Type: Full-Time

Salary: $150,000 - $160,000 per year plus bonus

Role Duties:

  • Participate in community outreach events and health fairs to raise awareness and provide health screenings
  • Coordinate referrals and follow-up care to ensure continuity of care for patients.
  • Provide culturally sensitive care and advocate for patients' rights and access to healthcare services.
  • Conduct comprehensive health assessments and screenings for individuals and families in the community
  • Collaborate with community organizations and stakeholders to develop and implement health education programs Stay updated with community health trends, evidence-based practices, and relevant Policies
  • Diagnose and manage acute and chronic medical conditions, including preventive care and health promotion.
  • Maintain detailed and accurate medical records in compliance with regulatory standards.

Qualifications Needed:

  • Master's degree in nursing from an accredited program.
  • Active state nursing license.
  • Current certification as a Nurse Practitioner.
  • Minimum of 3-5 years of clinical experience, preferably in community health or public health settings.
  • Strong interpersonal and communication skills, with the ability to work effectively with diverse populations
  • Commitment to social justice, health equity, and advocating for vulnerable communities.
