Nurse Practitioner Wanted

Position Summary:

The Nurse Practitioner provides nursing and medical services to patients, emphasizing health promotion and disease prevention, as well as the diagnosis and management of acute and chronic diseases. The FULL-TIME functions under established standardized protocols and works autonomously as well as in collaboration with other health care professionals to diagnose and manage client’s health care needs.

Salary: $150,000 to $160,000 per year plus bonus

Responsibilities:

• Conduct comprehensive health assessments, including physical exams and screenings, for a diverse patient population
• Diagnose and manage acute and chronic medical conditions, emphasizing preventive care and health promotion
• Develop and implement individualized treatment plans in collaboration with patients and multidisciplinary teams
• Provide culturally sensitive care and advocate for patients' rights and access to healthcare services
• Coordinate referrals and follow-up care to ensure continuity of care for patients
• Maintain accurate and up-to-date medical records in compliance with regulatory standards using electronic health record (EHR) systems
• Participate in community outreach events and health fairs to raise awareness and provide health education
• Stay current with internal medicine trends, evidence-based practices, and relevant healthcare policies

Requirements:

• Master's degree in Nursing from an accredited program.
• Current certification as a Nurse Practitioner.
• Active state Nursing license.
• Minimum of 3-5 years of clinical experience, preferably in community health or public health settings.
• Strong interpersonal and communication skills, with the ability to work effectively with diverse populations.
• Commitment to social justice, health equity, and advocating for vulnerable communities.
