Responsibilities:

  • Function autonomously to care for patients with complex, acute, critical and chronic health conditions.
  • Prescribe medications including the frequency and dosage
  • Work individually and in interdisciplinary teams of resources and consultants
  • Facilitates patient transition within and between health care organizations to ensure accurate communication among the health care providers
  • Record and maintain patient records in timely manner to ensure all services are accurately accounted for and available for review by the interdisciplinary teams
  • Talk to patients about effectively managing their health, as well as assisting in designing appropriate treatment plans and recommending further care.

Qualifications:

  • Current Nurse Practitioner certification.
  • Active state nursing license.
  • Master's degree in nursing from an accredited program.
  • A minimum of 3 years of clinical experience in a relevant specialty.
  • Exceptional communication and interpersonal skills.
  • Strong analytical and problem-solving abilities.
  • Proficiency in both Spanish and English.
