Job Description:

As a Family Nurse Practitioner, you will play a vital role in providing primary healthcare services to children and adults. Working in collaboration with a multidisciplinary team, you will assess, diagnose, and treat patients across the lifespan, from newborns to adolescents, with a focus on preventive care, health promotion, and disease management.

Responsibilities:

  • Conducting comprehensive health assessments and physical examinations
  • Ordering, performing, and interpreting diagnostic tests to inform diagnosis and treatment plans
  • Collaborating with the care team to develop and implement patient-centered care plans
  • Prescribing medications and providing appropriate treatments
  • Providing health education and counseling to patients and families
  • Participating in team meetings and case conferences to discuss patient progress and coordinate care
  • Documenting patient encounters and maintaining accurate medical records
  • Collaborating with other healthcare professionals to deliver holistic care
  • Contributing to quality improvement initiatives to enhance patient outcomes

Requirements:

  • Valid and unrestricted Nurse Practitioner license in New York
  • Minimum of 2 year of experience as a Family Nurse Practitioner
  • Strong knowledge of healthcare practices, procedures, and protocols
  • Excellent clinical assessment and diagnostic skills
  • Proficiency in interpreting diagnostic tests and formulating treatment plans
  • Effective communication and counseling skills
  • Strong organizational and time management abilities
  • Proficiency in electronic health record (EHR) systems
