Nurse Practitioner | Family Medicine

Location: Arizona State Only!

Salary Range: $130,000 - $150,000

What we are looking for:

We are seeking an innovative and passionate full-time Nurse Practitioner (NP) to join our team. We are outcomes driven and have a strong focus on the integrated care of the patient through our partnerships with counseling/behavioral health facilities. We have been watching the substance abuse epidemic grow around the nation, and we are motivated to provide patient centered services that will make a difference. We are looking for someone who has the enthusiasm, creativity, and flexibility to join us in redefining the industry.

Employment Type: Full-time

Schedule: Monday – Friday (9AM – 5PM)

How you will make an impact:

  • Deliver primary care services, with a client-centered, harm-reduction focus.
  • Referring patients to specialists, hospitals, community agencies or in-house staff as needed.
  • Ensure that appropriate laboratory studies have been performed and reviewed, as needed.
  • Ensure that a medical evaluation, including a medical history and physical examination, has been performed at admission and annually.
  • Completes physical examinations, procedures lab specimens and records the results of the test, presents medical
  • Collaborate with the multidisciplinary team to assist with quality improvement and development of creative strategies to meet client needs.
  • Advocate for individual client needs to ensure access to effective care, follow-up and resources from both an individual and systemic level.

What we need:

  • Master’s degree in nursing from an accredited college or university
  • Active NP license in Arizona State.
  • Completion of a Nurse Practitioner program specific to the expanded specialty of family medicine
  • 3 years of experience as a NP in medication assisted treatment and family medicine
  • Experience working in a team of healthcare professionals and collaborating with them to ensure positive, effective patient experiences with our organization.
  • Excellent communication and inter-personal skills.
