Full-time/Part-time Family Nurse Practitioner Needed at Southern Healthcare Center in West Hempstead, NY

Southern Healthcare Center is currently seeking a dedicated and skilled Family Nurse Practitioner who is passionate about providing exceptional outpatient primary care services.

Our areas of focus include Primary Care for Families and Adults, Preventive Medicine and Wellness Promotion, Chronic Condition Management, and Telehealth Services.

Weekly Schedule:
Monday, Wednesday & Friday: 8:00a.m. – 5:00p.m.
Full-Time, but can be flexible with schedule

Responsibilities:

  • Provide comprehensive primary care services to patients.
  • Conduct onsite health risk assessments to identify potential health issues.
  • Deliver wellness education to promote preventive measures and healthy lifestyle choices.
  • Support individuals in making positive lifestyle changes for improved well-being.
  • Assist individuals in effectively managing their chronic conditions.
  • Oversee and ensure quality care provided by the medical staff at Marathon Health Center.
  • Foster a collaborative relationship with the center's staff for successful outcomes.

Qualifications:

  • Master’s Degree in Nursing License
  • Family Nurse Practitioner License Compliance
  • Ability to be knowledgeable and comply with Company standards of operations.
  • The ability to maintain friendly, cordial relations with all clients and employees; maintain a positive work atmosphere by acting and communicating in a manner that results in a positive work relationship with customers, co-workers and managers.
  • The ability to consult with patients through virtual communication channels.
  • Proven experience working as a Telehealth Clinician.
  • Ability to work efficiently in a fast-paced and autonomous environment.

Salary:

$120,000 - $130,000 per year
