Position Details:

• Type: Full-time
• Schedule: Monday to Friday, 9:00 AM – 6:00 PM | Two Saturdays per month, 9:00 AM – 5:00 PM
• Salary: $140,000 - $160,000 a year with full benefits
• Patient Volume: Up to 20 patients per day
• Location: Brooklyn, NY. (3) Offices. 1 office in Bay Ridge, Brooklyn, NY, East Flatbush, Brooklyn, NY, and 1 in East New York, NY. Will Travel to 1 office on a different day.

Key Responsibilities:

• Perform comprehensive assessments and develop individualized care plans tailored to
• manage a range of acute and chronic conditions.
• Collaborate with multidisciplinary team members to deliver high-quality, coordinated care.
• Educate patients and families, providing tools to navigate their health and wellness journeys effectively.
• Maintain accurate and timely documentation using the Athena EHR system.

Qualifications:

• Active NYS APRN License and DEA registration.
• Master of Science in Nursing (MSN) with FNP, ANP, or AGNP Board Certification.
• Minimum 2 years of experience in a clinical setting.
• Proficiency with Athenahealth EHR and familiarity with ICD-9/ICD-10 coding systems.
• Ability to perform phlebotomy procedures and provide inhalation therapies.
• Strong interpersonal, communication, and organizational skills.
• Ability to excel in a fast-paced environment.
