Work Schedule:

  • 9:00 a.m. to 6:00 p.m., with occasional extended clinics until 8:00 p.m.
  • Weekend participation in community outreach events or health fairs may be required.
  • Average workweek totaling around 40 hours.


Responsibilities:

  • Perform comprehensive health assessments and screenings for individuals and families within the community.
  • Diagnose and manage acute and chronic medical conditions, emphasizing preventive care and health promotion.
  • Collaborate with local organizations and stakeholders to devise and execute health education initiatives.
  • Deliver culturally sensitive care while championing patients' rights and accessibility to healthcare services.
  • Coordinate patient referrals and ensure seamless follow-up care for enhanced patient outcomes.
  • Maintain meticulous and compliant medical records.
  • Engage in community outreach events and health fairs to foster awareness and deliver health screenings.
  • Stay abreast of community health trends, evidence-based practices, and pertinent policies.


Requirements:

  • Valid Nurse Practitioner certification.
  • Active state nursing license.
  • Master's degree in nursing from an accredited institution.
  • 3-5 years of clinical experience, preferably within community health or public health domains.
  • Exceptional interpersonal and communication abilities, adept at working with diverse communities.
  • Dedication to social justice, health equity, and championing the needs of vulnerable populations.
