Work Schedule:

  • Monday to Friday: 9:00 a.m. to 6:00 p.m., with occasional extended hours until 8:00 p.m.
  • Participation in weekend community outreach events or health fairs may be required.
  • Average weekly commitment of approximately 40 hours.


Key Responsibilities:

  • Conduct comprehensive health assessments and screenings for individuals and families.
  • Diagnose and manage acute and chronic medical conditions, prioritizing preventive care and health promotion.
  • Collaborate with local organizations to develop and implement health education programs.
  • Provide culturally sensitive care while advocating for patient rights and healthcare accessibility.
  • Facilitate patient referrals and ensure coordinated follow-up care for optimal outcomes.
  • Maintain accurate and compliant medical records.
  • Participate in community outreach events and health fairs to raise awareness and offer health screenings.
  • Stay updated on community health trends, evidence-based practices, and relevant policies.


Qualifications:

  • Valid Nurse Practitioner certification and active state nursing license.
  • Master's degree in nursing from an accredited institution.
  • 3 years of clinical experience, preferably in community or public health settings.
  • Strong interpersonal and communication skills, with the ability to work effectively with diverse communities.
  • Commitment to social justice, health equity, and serving vulnerable populations.


Compensation and Benefits:
