Work Schedule:

• Monday to Friday: 9:00 a.m. to 6:00 p.m., with occasional extensions to 8:00 p.m.
• Availability for weekend community outreach events or health fairs when needed.
• Consistent commitment averaging around 40 hours weekly.

Key Responsibilities:

• Perform thorough health assessments and screenings for individuals and families.
• Address acute and chronic medical conditions with an emphasis on preventive care and health promotion.
• Collaborate with community organizations to design and execute health education initiatives.
• Deliver culturally competent care while championing patient rights and healthcare accessibility.
• Coordinate patient referrals, ensuring seamless continuity of care.
• Uphold meticulous and compliant medical record-keeping.
• Engage actively in community health events to heighten awareness and provide screenings.
• Stay abreast of evolving community health trends, evidence-backed practices, and pertinent policies.

Qualifications:

• Active Nurse Practitioner certification and state nursing license.
• Master's degree or Doctorate in Nursing from a recognized institution.
• A minimum of 3 years of clinical experience, with a preference for community or public health roles.

Compensation & Benefits:
