We're seeking a compassionate Nurse Practitioner eager to make a positive impact on the health and well-being of underserved populations. This role involves offering comprehensive care, collaborating with local groups, and spearheading health education projects.
Work Schedule:
- Full-time position (approximately 40 hours per week)
- Monday to Friday, 9:00 a.m. to 6:00 p.m. (occasional evening clinics until 8:00 p.m.)
- Participation in weekend community outreach events or health fairs may be required
Responsibilities:
- Conduct comprehensive health assessments and screenings for individuals and families
- Diagnose and manage acute and chronic medical conditions, including preventive care
- Collaborate with community organizations for health education programs
- Provide culturally sensitive care and advocate for patients' rights and healthcare access
- Coordinate referrals and follow-up care to ensure continuity for patients
- Maintain accurate medical records in compliance with regulatory standards
- Participate in community outreach events and health fairs to raise awareness and provide screenings
- Stay updated with community health trends, evidence-based practices, and policies
Requirements:
- Current Nurse Practitioner certification and active state nursing license
- Master's degree in nursing from an accredited program
- 3 years of clinical experience, preferably in community or public health settings
- Strong interpersonal and communication skills, working effectively with diverse populations
- Commitment to social justice, health equity, and advocating for vulnerable communities
Salary and Benefits:
