We're seeking a compassionate Nurse Practitioner eager to make a positive impact on the health and well-being of underserved populations. This role involves offering comprehensive care, collaborating with local groups, and spearheading health education projects.

Work Schedule:
  • Full-time position (approximately 40 hours per week)
  • Monday to Friday, 9:00 a.m. to 6:00 p.m. (occasional evening clinics until 8:00 p.m.)
  • Participation in weekend community outreach events or health fairs may be required

Responsibilities:
  • Conduct comprehensive health assessments and screenings for individuals and families
  • Diagnose and manage acute and chronic medical conditions, including preventive care
  • Collaborate with community organizations for health education programs
  • Provide culturally sensitive care and advocate for patients' rights and healthcare access
  • Coordinate referrals and follow-up care to ensure continuity for patients
  • Maintain accurate medical records in compliance with regulatory standards
  • Participate in community outreach events and health fairs to raise awareness and provide screenings
  • Stay updated with community health trends, evidence-based practices, and policies


Requirements:
  • Current Nurse Practitioner certification and active state nursing license
  • Master's degree in nursing from an accredited program
  • 3 years of clinical experience, preferably in community or public health settings
  • Strong interpersonal and communication skills, working effectively with diverse populations
  • Commitment to social justice, health equity, and advocating for vulnerable communities
