We're in search of a compassionate Nurse Practitioner eager to positively impact the health and wellness of underserved populations. This role involves delivering comprehensive care, collaborating with local organizations, and spearheading health education initiatives.
Work Schedule:
- Full-time position (around 40 hours per week)
- Monday to Friday, 9:00 a.m. to 6:00 p.m. (occasional evening clinics until 8:00 p.m.)
- Possible participation in weekend community outreach events or health fairs
Responsibilities:
- Perform thorough health assessments and screenings for individuals and families
- Diagnose and manage acute and chronic medical conditions, including preventive care
- Partner with community organizations for health education programs
- Provide culturally sensitive care and advocate for patients' rights and healthcare access
- Coordinate referrals and follow-up care to ensure patient continuity
- Maintain accurate medical records in compliance with regulations
- Engage in community outreach events and health fairs for awareness and screenings
- Stay updated on community health trends, evidence-based practices, and policies
Requirements:
- Current Nurse Practitioner certification and active state nursing license
- Master's degree in nursing from an accredited program
- 3 years of clinical experience, preferably in community or public health settings
- Excellent interpersonal and communication skills, adept at working with diverse populations
- Dedication to social justice, health equity, and advocating for vulnerable communities
Salary and Benefits:
