Health Nurse Practitioner Needed
Salary: $150,000 to $160,000
Schedule: Monday to Friday 9-5 pm

Responsibilities:
- Conduct comprehensive health assessments and screenings for individuals and families in the community.
- Diagnose and manage acute and chronic medical conditions, including preventive care and health promotion.
- Collaborate with community organizations and stakeholders to develop and implement health education programs.
- Provide culturally sensitive care and advocate for patients' rights and access to healthcare services.
- Coordinate referrals and follow-up care to ensure continuity of care for patients.
- Maintain detailed and accurate medical records in compliance with regulatory standards.
- Participate in community outreach events and health fairs to raise awareness and provide health screenings.
- Stay updated with community health trends, evidence-based practices, and relevant policies.
Requirements:
- Master's or Doctoral degree in Nursing from an accredited program
- Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) by the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP)
- Current, unrestricted Nurse Practitioner (APRN) license in New York State
- DEA# and NPI# are required
- Experience in primary care or family medicine preferred
- Strong clinical assessment and diagnostic skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Proficiency in using electronic health records (EHR) systems
- Minimum of 2-5 years of clinical experience, preferably in community health or public health settings.
- Strong interpersonal and communication skills, with the ability to work effectively with diverse populations.
