Salary:
$120,000 - $130,000 annually


The perfect candidate will have thorough knowledge and understanding of geriatric mental health. To excel in this role you should be personable, service-oriented, and have professionalism of the highest caliber with exemplary communication. Strong attention to detail is required as this position routinely handles patient information in electronic medical records.

Responsibilities:

  • Diagnosing and treat behavioral and psychiatric health.
  • Conducting individual, group, and family counseling sessions.
  • Providing psychotherapy to patients.
  • Managing patient medication.
  • Diagnosing and develop treatment plans for homebound patients via analyzing and interpreting medical histories, symptoms, physical findings, or diagnostic imaging.
  • Maintaining standards of care and professional practice guidelines.
  • Collaborating with our team of specialists and primary care providers to develop treatment plans for high-risk patients.
  • Partnering with local home health agencies, labs, imaging companies, and vendors.
  • Communicating with patients and caregivers regarding self-management of diagnoses.
  • Responding to patient and caregiver questions in a timely and knowledgeable manner.
  • Maintaining compliance with HIPPA and Health Care regulations.



Interested candidates should have:

  • One year of home health experience preferred, but not mandatory.
  • Willingness to travel 100% of the time.
