The perfect candidate will have thorough knowledge and understanding of geriatric mental health. Strong attention to detail is required as this position routinely handles patient information in electronic medical records.

Responsibilities:

  • Diagnosing and treating behavioral and psychiatric health.
  • Conducting individual, group, and family counseling sessions.
  • Providing psychotherapy to patients.
  • Managing patient medication.
  • Diagnosing and developing treatment plans for homebound patients via analyzing and interpreting medical histories, symptoms, physical findings, or diagnostic imaging.
  • Maintaining standards of care and professional practice guidelines.
  • Collaborating with our team of specialists and primary care providers to develop treatment plans for high-risk patients.
  • Partnering with local home health agencies, labs, imaging companies, and vendors.
  • Communicating with patients and caregivers regarding self-management of diagnoses.
  • Responding to patient and caregiver questions in a timely and knowledgeable manner.
  • Maintaining compliance with HIPPA and Health Care regulations.

Requirements:

  • Current certification as a Nurse Practitioner.
  • Active state nursing license.
  • Master's degree in nursing from an accredited program.
  • Minimum of 3-5 years of clinical experience, preferably in occupational health or related field.
  • Strong knowledge of occupational health regulations and practices.
  • Excellent communication and problem-solving skills.
