Responsibilities:

  • Provides continuous comprehensive medical and nursing care, including preventive and supportive health care services to patients and families.
  • Manages health problems and coordinates health care for various patient populations. Identifies and diagnoses illnesses and physical conditions, and implements therapeutic and corrective measures.
  • Consistent with the established nursing practice, differentiates between normal and abnormal findings in the screening process to detect changes in health status that require physician’s intervention.
  • Develops and maintains partnerships with community-based organizations and makes referrals to consulting physicians and/or other health care professionals and community agencies, as necessary.
  • Evaluate and interpret patient data, in order to plan and implement appropriate nursing interventions.
  • Consults with physicians and other health care practitioners in the evaluation of health screening findings to make prospective decisions about treatment that will best support improved outcomes and patient satisfaction.
  • Formulates differential diagnosis and the plan of care based on medical history, physical exams, nursing assessment, and other pertinent information. Identifies health and medical risks and needs.
  • Develop, implement, and evaluate patient plan of care. Determines effectiveness and modifies as per clinical standards and practice protocols, where needed, to achieve medical and health goals.
  • Orders, conducts and interprets diagnostic and laboratory tests. Identifies and may prescribe appropriate pharmacological and nonpharmacological interventions and therapies.
  • Manages a patient session schedule allowing for both scheduled and non-emergent unscheduled visits; maintains a work schedule that ensures availability and continuity of care; ensures optimal availability and utilization of appropriate resources.
  • Provides education, guidance, and direction to nursing staff and other members of the health care team to ensure quality and safe medical and nursing care is provided.
  • Provides education and counseling to patients and families regarding health status, treatment and progress, medication management, disease prevention, and health promotion, records, and maintains a system of relevant documentation.
  • Participates in internal/external quality assurance/performance improvement (QA/PI) activities, programs, and training, as required.

Qualifications

  • A valid State license and current registration to practice as a Registered Professional Nurse; and
  • Current certificate as a Nurse Practitioner issued by the State Department of Education; and
  • A current certification issued by the State Education Department to prescribe medication.
