Schedule: Monday to Friday, 9 AM – 5 PM

Compensation: $85,000 to $90,000 + benefits


Position Overview:

We’re seeking a dedicated and skilled Occupational Therapist to join our rehab team and provide meaningful, resident-centered care. As an Occupational Therapist, you will assess, plan, and implement individualized therapy interventions designed to improve functional abilities in activities of daily living (ADLs), fine motor skills, cognitive tasks, and upper extremity function. You will work with residents recovering from illness, injury, surgery, and chronic conditions, collaborating closely with other therapists, nurses, and care staff.

Job Duties:

  • Conduct evaluations and create personalized treatment plans based on resident needs and goals
  • Deliver therapeutic interventions to enhance ADLs, upper body function, and cognitive ability
  • Train residents in adaptive equipment use, safety techniques, and energy conservation strategies
  • Document evaluations, treatment progress, and outcomes in the EMR system
  • Collaborate with interdisciplinary care teams including PTs, SLPs, nurses, and physicians
  • Educate residents and families on techniques to support long-term function and independence
  • Maintain compliance with state, federal, and facility regulations

Requirements:

  • Licensed Occupational Therapist in New York State (required)
  • Graduate of an accredited Occupational Therapy program (Master’s or Doctorate)
  • NBCOT certification
  • Current CPR/BLS certification
  • Strong communication, problem-solving, and organizational skills
  • Passion for working with older adults and medically complex patients

Preferred Qualifications:

  • 1+ year of experience in a skilled nursing facility or geriatric setting
  • Knowledge of PDPM and Medicare documentation guidelines
  • Familiarity with EMR systems (e.g., PointClickCare, NetHealth, Rehab Optima)
  • Experience with upper extremity rehab, cognitive retraining, and discharge planning
