Job Title: Full-Time Pharmacist – Hospital-Based Clinical Pharmacy (Arizona)
Location: Phoenix , Arizona
Job Type: Full Time
Work Hours: Monday - Friday: 9 AM-5 PM
Compensation: $120,000.00 - $150,000 per year + benefits
As a Hospital Pharmacist, you will provide comprehensive pharmaceutical care to inpatients across multiple units. You’ll collaborate with physicians, nurses, and allied health professionals to manage complex medication regimens, support critical care initiatives, and ensure compliance with state and federal regulations.
Your work will influence everything from emergency interventions to chronic disease management, and you'll play a key role in optimizing patient recovery, preventing adverse events, and promoting medication adherence.
Core Responsibilities:
- Interpret, verify, and process medication orders for safety, efficacy, and appropriateness
- Monitor and adjust medication therapy based on lab values, pharmacokinetics, renal/hepatic function, and therapeutic drug monitoring protocols
- Provide real-time consults and drug information to physicians and nurses
- Participate in interdisciplinary rounds in ICU, med-surg, ED, oncology, and specialty units
- Compound and dispense medications accurately, including sterile IV admixtures and chemotherapy agents
- Oversee automated dispensing systems (e.g., Pyxis) and medication inventory management
- Ensure compliance with hospital protocols, The Joint Commission standards, USP 797/800, and state/federal laws
- Support pharmacy students or residents (if in a teaching hospital setting)
- Contribute to formulary management, policy development, and quality improvement projects
- Participate in code blue responses and emergency drug delivery when needed
Qualifications:
- Doctor of Pharmacy (PharmD) from an ACPE-accredited institution
- Current Pharmacist license (or eligible) in the state of Arizona
- Completion of a PGY1 Pharmacy Residency preferred (PGY2 a plus for clinical or specialty roles)
- Strong clinical knowledge and experience in hospital/acute care settings
- Familiarity with EHR systems (e.g., Epic), automated dispensing cabinets, and pharmacy software
- Excellent communication, problem-solving, and interpersonal skills
- Certification in ACLS/BLS or willingness to obtain upon hire
- High attention to detail and commitment to patient safety
