Position: Physical Therapist (PT)
Work Setting: Skilled Nursing Facility (SNF)
Employment Type: Full-Time / Part-Time
Schedule: Monday to Friday (9 AM – 5 PM)
Compensation: $100,000 to $120,000 + benefits

Position Overview:
We are seeking a licensed Physical Therapist to provide exceptional rehabilitative care to residents in our skilled nursing facility. The ideal candidate will have a passion for improving patients’ mobility, function, and quality of life through tailored treatment plans.
Key Responsibilities:
- Evaluate and assess patients’ physical conditions and functional limitations.
- Develop and implement individualized treatment plans to promote recovery and enhance mobility.
- Administer therapeutic exercises, manual therapy, and modalities to address pain and physical impairments.
- Monitor and document patients’ progress, adjusting treatment plans as necessary.
- Collaborate with interdisciplinary teams, including physicians, nurses, and occupational and speech therapists, to optimize patient outcomes.
- Educate patients and families on home exercise programs and strategies for continued progress.
- Ensure compliance with facility protocols, state regulations, and best practices.
Requirements:
- Education: Doctor of Physical Therapy (DPT) or a Bachelor’s/Master’s in Physical Therapy from an accredited program.
- Licensure: Active and unrestricted Physical Therapy license in the state of New York.
- Experience: Previous experience in a skilled nursing facility, rehab center, or similar setting is preferred.
- Strong clinical, communication, and interpersonal skills.
