Physical Therapist – Assisted Living Facility (Trenton, New Jersey)
Employment Type: Full-Time / Part-Time
Schedule: Monday to Friday (9 AM – 5 PM)
Compensation: $90,000 to $100,000 + benefits

The Opportunity:

Are you a licensed Physical Therapist who thrives in a close-knit, residential care setting? We're seeking a compassionate and driven Physical Therapist to join our care team within our Assisted Living Facility (ALF) communities across New Jersey.

This role is perfect for therapists who value meaningful, ongoing relationships with older adults, want to provide personalized and impactful care, and are looking for a steady and supportive environment without the constant travel of traditional home health.

Who are we looking for?

  • Current New Jersey Physical Therapist (PT) license in good standing
  • Graduate of an accredited Physical Therapy program (DPT or equivalent)
  • Experience working with seniors or in an ALF/SNF/rehab setting is preferred
  • Excellent interpersonal and communication skills
  • Strong organizational skills and ability to work autonomously
  • Familiarity with electronic documentation (EMR systems)

What will you do?

  • Conduct initial evaluations and develop individualized treatment plans for residents
  • Deliver evidence-based physical therapy interventions to promote strength, balance, and mobility
  • Provide fall risk assessments and assistive device training
  • Collaborate closely with facility nursing staff, family members, and other disciplines to ensure comprehensive care
  • Monitor, document, and adjust therapy plans based on resident progress
  • Educate residents, caregivers, and staff on safe mobility and exercises
  • Participate in care planning and interdisciplinary team meetings
