We are seeking a dedicated Physical Therapist to join our interdisciplinary team at a premier Skilled Nursing Facility in New York, where patient-centered care, professional growth, and teamwork are not just ideals—they’re part of everyday practice.

As a Physical Therapist, you’ll be a key contributor to our residents’ recovery journeys. Your clinical skills, creativity, and passion for helping others will guide each patient through individualized care plans that promote mobility, strength, and overall well-being.

Job Duties:

• Conduct initial and ongoing assessments to evaluate resident mobility, pain levels, strength, and functionality.
• Develop and implement individualized treatment plans aligned with residents’ goals and physicians’ recommendations.
• Deliver evidence-based therapy sessions that focus on improving mobility, reducing pain, and preventing injury.
• Educate residents, families, and caregivers on therapeutic techniques and at-home exercises.
• Collaborate with nursing, occupational therapy, and wellness staff to create interdisciplinary care plans.
• Maintain accurate documentation in compliance with Medicare and state regulatory standards.
• Contribute to a positive, patient-focused environment that respects and enhances the dignity of our seniors.

Qualifications:

• Current NYS licensure as a Physical Therapist (required).
• Graduate from an accredited Physical Therapy program (DPT or equivalent).
• Prior experience in a skilled nursing or long-term care setting is preferred but not required.
• Strong interpersonal, communication, and clinical reasoning skills.
