Salary Range: $100,000 – $120,000 per year

Location: Eden , Texas

Job Type: Full-Time

Schedule: Monday to Friday, 9 AM – 5 PM

We are looking for a Physical Therapist who is excited about the opportunity to work with the elderly population and can make a real difference in their recovery journey. As part of our interdisciplinary team, you’ll be supported by a team of physicians, nurses, occupational therapists, and speech-language pathologists.

As a Physical Therapist in our SNF, your role will be to provide high-quality care focused on enhancing mobility, strength, and overall functionality for our elderly residents.

Key Responsibilities:

  • Patient Evaluation: Perform comprehensive evaluations of residents to assess physical impairments, functional limitations, and rehabilitation goals.
  • Treatment Plans: Develop and implement individualized, evidence-based treatment plans aimed at improving strength, mobility, and overall function.
  • Therapeutic Interventions: Utilize a variety of treatment techniques, including therapeutic exercises, manual therapy, functional mobility training, and modalities.
  • Progress Monitoring: Continuously assess patient progress and adjust treatment plans as needed to ensure optimal outcomes.
  • Collaborative Care: Work closely with the interdisciplinary healthcare team, including nurses, physicians, and occupational therapists, to deliver coordinated care and meet the overall goals of the patient.
  • Patient Education: Provide education to residents and their families on mobility techniques, injury prevention, and safe practices for activities of daily living (ADLs).
  • Documentation: Maintain accurate, timely, and compliant documentation of patient progress, treatment plans, and outcomes, in accordance with facility policies and regulatory standards.

Qualifications:

  • Licensing/Certification: Current, valid Physical Therapist license in the state of Texas (or the ability to obtain one).
  • Experience: Previous experience in skilled nursing, geriatrics, or rehabilitation settings is preferred. New grads with an interest in geriatric care are also encouraged to apply.
  • Skills: Strong knowledge of rehabilitation techniques, patient care protocols, and physical therapy modalities. Ability to assess and treat complex medical conditions commonly found in the elderly population.
  • Personality: Excellent communication skills, empathy, and a strong desire to make a positive impact on the lives of elderly patients.
  • Teamwork: Ability to collaborate effectively with a diverse healthcare team to ensure comprehensive patient care.
  • Work Ethic: Ability to manage a varied caseload while delivering personalized care with attention to detail.
