Full-Time Physical Therapist Needed

We are seeking a dedicated Physical Therapist to join our team at our Skilled Nursing Facility (SNF). In this role, you will provide high-quality rehabilitation services to residents, focusing on improving their physical function, mobility, and overall quality of life. You will collaborate with a multidisciplinary team to develop and implement personalized treatment plans tailored to each resident's needs.

Salary: $85,000-$95,000 per year with Full Benefits

Schedule:

  • Monday to Friday 9 AM to 5 PM (Flexible)
Patient Volume: 8 patients a day
Insurance Mix: Medicare & Major Medical Commercial Insurances

Responsibilities
  • Assess and evaluate physical abilities and limitations through comprehensive evaluations.
  • Develop and implement individualized treatment plans based on goals, diagnoses, and overall health status.
  • Utilize evidence-based interventions and techniques to enhance mobility, strength, and coordination.
  • Monitor progress, adjusting treatment plans as necessary to ensure optimal outcomes.
  • Educate patient and their families on injury prevention, wellness, and home exercise programs.
  • Collaborate with nurses, occupational therapists, speech therapists, and other healthcare professionals to provide holistic care.
  • Maintain accurate and timely documentation of treatment plans, progress notes, and discharge summaries in accordance with facility policies and regulatory standards.
  • Participate in interdisciplinary team meetings and contribute to case discussions and care planning.
  • Stay current with advancements in physical therapy practices and participate in ongoing professional development.
Qualifications
  • Master's or Doctorate Degree in Physical Therapy from an accredited institution.
  • Current New Jersey state license as Physical Therapist.
  • Must have 2 years of clinical experience as a Physical Therapist,
  • Strong manual therapy skills and training.
  • Familiarity with EMR Software (e.g., WebPT).
  • Exceptional interpersonal and communication skills.
  • Commitment to continuous learning and professional growth.
  • No disciplinary actions or board actions on license.
