Job Duties:
- Conduct comprehensive assessments to evaluate residents’ physical abilities and determine appropriate therapy goals.
- Develop and implement personalized treatment plans focused on restoring mobility, reducing pain, and enhancing functional independence.
- Deliver hands-on therapy, including therapeutic exercises, balance training, and mobility interventions.
- Educate residents, families, and caregivers on safe mobility techniques and home exercise programs.
- Collaborate with nurses, occupational therapists, and other healthcare professionals to ensure integrated and effective patient care.
- Maintain detailed documentation of treatment plans, progress reports, and outcomes in compliance with SNF regulations.
- Doctorate or Master’s degree in Physical Therapy from an accredited program.
- Current Virginia PT license in good standing.
- Experience in skilled nursing, geriatric, or long-term care settings preferred.
- Strong communication and interpersonal skills with a patient-centered approach.
- Ability to work effectively in a multidisciplinary healthcare team.
