Salary: $110,000 - $125,000 per year + benefits

Position Overview:
We are seeking a dedicated Homecare Physical Therapist to join our team in Missouri. As a Homecare Physical Therapist, you will provide personalized in-home physical therapy services to patients, helping them regain mobility, strength, and independence in the comfort of their own homes. Job Duties:
- Assess and evaluate patients' physical conditions and develop personalized treatment plans.
- Provide one-on-one physical therapy treatments and exercises in patients’ homes.
- Educate patients and their families on exercises, safety measures, and lifestyle modifications.
- Track patient progress and adjust treatment plans accordingly.
- Collaborate with healthcare teams, including physicians and nurses, to ensure holistic care for patients.
- Maintain accurate and up-to-date patient records, treatment plans, and progress notes.
- Ensure compliance with all regulatory and professional standards.
Requirements:
- Licensed Physical Therapist in the state of Missouri (Required).
- Minimum of 1 year of experience as a physical therapist, preferably in homecare or outpatient settings.
- Excellent communication and interpersonal skills.
- Strong clinical knowledge and ability to assess and treat a variety of conditions.
- Ability to work independently with strong time management skills.
- Valid driver’s license and reliable transportation for travel to patients' homes.
- Certification in geriatric physical therapy or other specialized areas.
- Experience working with elderly or homebound patients
- CPR and First Aid certification (Preferred but not required).
