Job Title: Homecare Physical Therapist

Location: Blue Springs, Missouri

Job Type: Full-Time / Part-Time

Salary: $110,000 - $125,000 per year + benefits

Position Overview:

We are seeking a dedicated Homecare Physical Therapist to join our team in Missouri. As a Homecare Physical Therapist, you will provide personalized in-home physical therapy services to patients, helping them regain mobility, strength, and independence in the comfort of their own homes. Job Duties:

  • Assess and evaluate patients' physical conditions and develop personalized treatment plans.
  • Provide one-on-one physical therapy treatments and exercises in patients’ homes.
  • Educate patients and their families on exercises, safety measures, and lifestyle modifications.
  • Track patient progress and adjust treatment plans accordingly.
  • Collaborate with healthcare teams, including physicians and nurses, to ensure holistic care for patients.
  • Maintain accurate and up-to-date patient records, treatment plans, and progress notes.
  • Ensure compliance with all regulatory and professional standards.

Requirements:

  • Licensed Physical Therapist in the state of Missouri (Required).
  • Minimum of 1 year of experience as a physical therapist, preferably in homecare or outpatient settings.
  • Excellent communication and interpersonal skills.
  • Strong clinical knowledge and ability to assess and treat a variety of conditions.
  • Ability to work independently with strong time management skills.
  • Valid driver’s license and reliable transportation for travel to patients' homes.
  • Certification in geriatric physical therapy or other specialized areas.
  • Experience working with elderly or homebound patients
  • CPR and First Aid certification (Preferred but not required).
