Job Opening: Homecare Physical Therapist (PT)

Location: East Brunswick, New Jersey

Job Type: Full-time/Part-time

Work Hours: Monday to Friday, 9 AM – 5 PM

Compensation: $90,000 - $110,000 per year + benefits (Full-Time)


About the Job:

We’re currently seeking a Licensed Physical Therapist who is passionate about providing personalized care and values the autonomy and flexibility that home health offers.

As a Homecare Physical Therapist, you will evaluate, treat, and support patients across a variety of conditions—post-surgical, orthopedic, neurological, and more. You’ll create custom treatment plans, educate patients and families, and track progress, all while enjoying a flexible schedule and the chance to truly connect with your patients.


What You’ll Do:

  • Conduct in-home physical therapy evaluations and develop individualized treatment plans
  • Provide therapeutic exercises, gait training, balance activities, and functional mobility interventions
  • Educate patients and caregivers on home safety, fall prevention, and wellness strategies
  • Monitor progress and adjust plans based on patients’ functional goals
  • Accurately document all visits in accordance with Medicare and agency guidelines
  • Communicate with the interdisciplinary team (nursing, OT, SLP, social work) for coordinated care

What Makes You a Great Fit:

  • Active New Jersey Physical Therapist License (required)
  • Graduate of an accredited PT program (DPT or MPT)
  • Valid driver’s license, reliable vehicle, and auto insurance
  • CPR/BLS certification
  • Strong clinical judgment and time-management skills
  • Comfortable working independently in a home setting
  • 1+ year of experience in home health, outpatient, or hospital-based rehab
  • Familiarity with EMR systems (e.g., HomeCare HomeBase, Kinnser, MatrixCare, etc.)
  • Knowledge of Medicare documentation and OASIS (preferred, not required—training available)
