Job Type: Full-Time / Part-Time

Salary: $110,000 - $120,000 per year + benefits

Position Summary:

We’re seeking a skilled Homecare Physical Therapist to join our dedicated, patient-centered team in New Jersey.

Job Duties:

  • Conducting in-home assessments to evaluate patients' physical conditions and functional limitations
  • Developing individualized care plans with measurable goals in alignment with physician directives
  • Delivering hands-on therapy and guiding patients through exercises to improve strength, mobility, and endurance
  • Educating patients and caregivers on therapeutic techniques and home exercise programs
  • Monitoring and documenting patient progress, adjusting plans of care as needed
  • Collaborating with nurses, occupational therapists, social workers, and physicians as part of a multidisciplinary care team
  • Ensuring compliance with federal, state, and agency regulations and documentation standards

Requirements:

  • Licensed Physical Therapist (PT) in the state of New Jersey
  • Graduate of an accredited Physical Therapy program (DPT or MSPT)
  • Minimum of 1 year of clinical experience preferred (home health experience is a plus)
  • Strong interpersonal and communication skills
  • Ability to work independently and manage a flexible caseload
  • Valid driver's license, reliable vehicle, and auto insurance
  • Familiarity with electronic documentation systems (e.g., Kinnser, Homecare Homebase, or similar)
