Responsibilities:
- Provide evaluation services including:
- Reviewing pertinent history information
- Performing specific assessments
- Determining recommendations and documenting results.
- Explain evaluation findings and treatment plans to patients and families, involving them in goal setting.
- Demonstrate proficiency in implementing treatment protocols based on evaluation findings, patient presentation, and the evolving healthcare environment.
- Consult with other healthcare professionals as indicated.
- Recognize and document changes in the patient's condition requiring modification of the plan of care and/or the need for medical/rehabilitation consultation.
- Complete appropriate discharge planning and follow-up care.
- Accurately and timely document and charge for patient care activities, painting a clear picture of the patient's course of care including:
- Focused initial evaluation/plan of care establishing baselines and goals
- Legible daily encounter notes
- Interim progress summaries
- Discharge evaluation.
- Demonstrate effective time management and meet productivity expectations.
- Maintain an accurate schedule, be timely and punctual for meetings, and participate in hospital and community-based marketing activities.
- Contribute to the development and achievement of established department goals and objectives.
- Comply with governmental and accreditation regulations.
- Take initiative in pursuing and directing continuing education for professional growth and competency for self and organization.
- Serve as a clinical instructor to assign students.
- Promote excellent customer experience and great access to care through flexibility and adaptability.
- Participate in the team's removal of barriers to access, understand CSI measurement data of the sites, and actively engage in the Access NRH initiatives.
- Bachelor's degree in Physical Therapy
- High School Diploma or GED from an accredited school of Physical Therapy
- State license required as deemed necessary for your specific location(s)
- American Heart Association's BLS for Healthcare Providers CPR certification required
- Prior work experience is not required
Schedule:
- Monday to Friday
- $100,000 - $120,000 a year plus full benefits.
